i tried triggering the maggots scene in hotb and could not but whatever, also game was surprisingly short huh
and by the way, the orcs were great, their bearhug was nigh impossible to escape which gave a sense of realistic bearhug
fucking sad that they did not have a rescue failure scene tho
those were kinda cool albeit short

the guy with the sword's name was goto, and in hotm it is the gang's name, right?

I think goto is more like an easter egg connection than a lore part.
As for HotB in general, well...game was going in circles for years so as sad as it was, it's good it was killed for a new project. There was a risk that a new project would be going in circles even more because very first iterations/demos were something super weird I think...like top-down rogue-like or something.
You only earn hrank by killing appropriate enemies.
Some places you need to advance the story to unlock.
I think rank 3 is highest at the moment.

One thing i would suggest is shorting the time it takes to trigger the defeat scene with the ants. It takes is very long time to activate.

The last one (007) is definitely the bad ending for constantly resisting the "rescue" party. After you break their blackmailing scheme, the next time you lose to them, you will be sold to the VIP client (the fat guy).
I can only repeat myself, but Inui does a hell of a job with this game and it really shapes to something great. But he really should look for himself FIRST and than after his game, the fans will appreciate it. It is like painting a picture while being dangerously close to doze off.
But when I read this correctly, than he doesn't make the models himself, it seems and rather use assets for his game, which is fine by me. So here is a thought, if the big villain is using the drug on himself, I would suggest to use something really big and special, like a dragon or colosal monster for example. As for Lui... Luka... darn it, I forgot his name (the guy who tries to push you into Prostitution)... I could imagine him transforming into a fat pig. And as for the ork-like guy, maybe something like Raijin.
Another idea could be plants, that use suction cups to milk the MC. I mean, with such big boobs, this is a must. Or maybe a machine. But yeah, that one is a personal kink of myself

No, right now, when estrus is high, it gets only more difficult to escape enemies grabs. I believe once it is as high as 30+
