So, a more accurate description, since a lot of people (men mostly, especially in porn writing) don't know this. Letter based breast sizes are bra sizes, which have 2 measurements. The first one is the band size, measured around the torso just below the breasts. The second measurement is around the breasts at the widest part. So, if the band is 36" and the breast is 42" there's a 6 inch difference between them, which determines the cup size. A cup is 1 inch difference, B cup is 2", C cup is 3", exc with DD replacing F (for some reason). In the example numbers I made it's a 36 F.
