Some species are not implemented to be reborn as. It's not a 100 % reborn either, more of a crossbreed. You will have to wait then. Iirc it matters if the baby stays with Lona or runs away after birth.

If you gave birth to 3 fishkinds at once, you chose the wrong pregnancy. You need to give birth to a "human" via Deep One impregnation. As for the Sea Witch, you can find her in the dungeon in the middle of Fishtopia isles. Those two particular achievements are related to rebirthing. Basically, you need to talk to the Sea Witch as a Deep One and the Abomination god as an abomination. In the case of the Deep One, you need to specifically activate the skill you get before you see the achievement.
I think the babies no longer run away as of 0.6.7. Changelog states

As far as I know, Rebirth is only possible as a Human, Moot, Abomination and Deep One.
You must have Lona give birth to one of these before the option to be reborn is possible.

There are only warmups and a fight with The Beast. From what i know it's random and quite rare.
Fortunately from what I remember the fight is determined only when you go in so you can save and reload until you get the beast. (of course if you don't mind save scumming)

I think some information is either missing or incomplete. Like giving birth to a human is not guaranteed if you're playing another race. It'll be especially hard to tell if you're playing a Deep One. Goblins have a low chance of producing a Moot too. At least for me, Human (Abomination) for me was the very last option of 3 "human"s. You could also carry 10 pieces of semen and save the trip if you already have the Sea Witch CG.
You also haven't discussed taking the boat as an alternative to simply getting Lona killed. I haven't played a fresh game since 0.6.6, but the benefit of taking the boat is an extra trait point (2 points, instead of 1). You also get additional trait points for subsequent rebirths, regardless of whether you take the boat or not, though taking the boat ending more than once may not give you extra points.
You should also combine True Deep One with regular Deep One, since you can switch between both in the same playthrough.
P.S. I strongly advise against giving people the option to be enslaved. You'll basically lose access to the bank, Noer's (and possible other) Inn, and be attacked by guards on sight. The cost to remove the brand is 10k TP, if I remember right, which may difficult to earn for newer players.
You should also specify that parasitic abominations/births don't count. They produce more parasites rather than a baby.

I didn't really want to make a full guide but just wanted to compile some information. When I was searching for rebirth information a while ago there was a lot of confusion and it took a lot of clicking.

Cause I made post with the latest translations a while back. Feel free to use that instead. It isn't perfect but it is what it is. There was a post a few pages back with MTL program and stuff that you could probably find.
