If you're going the love route that would be NTR and thus not included in the game. Unless there's going to be some wreck it Ralph porn game where games cross over. Getting on some inception level of story telling

The doctor if I am remembering right had the crazy eyes too. So it's possible she did something to save the MC. Could be spoiling the story or just making speculations.
