You're not missing much if you don't play the game. Basic trainer elements with the brothel and some fight scenes are about it, no actual story. In the end, it just comes down to going through the motions and a bit of grinding until you get to the sex scenes.
My personal advice is to make sure you're not horny as much when playing games such as this. I'd would usually jack off or have sex with someone earlier on where I'm like whew!
