hey, i am deciding whether or not to come back to this game. the last time i played was the orc village quest years ago. some question does the nameless finally get more story or at least do we know the consequences of submitting to him? also how recommended is it to play the game after that point? does it finish old story line or just new wip over new wip?

Nope, just new wip over new wip. Old stories barely get touched anymore.
Tho there are tons of hot sex scenes (with CGs) got added which is quite nice for a quick fap session.

There has been no added story since like 3years ago, just random bits where you don't even play as your own char

So...Did Virile write the new Barrel scene with AI?
It suddenly becomes a cringe "Daddy say I say Daddy say I say" dialog tree that yapping on and on, you usually see those written by horny 10yr olds. Probably the most cringe scene I read in LD and none of other scenes I would even associate with the word.
