I may be misunderstanding you, but I don't think having Ichigo be the protagonist would change much. Any Bleach game with dating/sex elements would surely include Rukia unless it was like a kinetic game focused on one girl. If it were set prior to Rukia and Renji's relationship, I don't think that would be NTR or much different than the female LI getting with some random schmuck.

I mean you're right it wouldn't change much, but still it's better for it to be Ichigo than some random dude because there's already that romantic connection there for it to make sense. Also I believe Ichigo should've gotten with Rukia anyway.
