err if we talking you try avoid it or make it happen, more or less possible just never use heavy attack to kill enemies since its always end up as mutilating enemies also never craft flesh bench since that where most guro stuff came from but if we talking about the environment and surrounding area, nope some area had head stabbed in pike as display and one of boss literally using females bodies as weapon hanging in his back
