To be fair, player choice can destroy a NTR game unless you do it juuuuuuust right. The best way to go about it imo is having the MC being competent but not use to other women besides main heroine. Then you add in a second heroine who suddenly seem interested in MC, and while MC is distracted with this new girl the antagonist starts making moves on the main heroine.This is one of my biggest annoyances with NTR games as well. Assuming it isn't the worst kind of NTR game where you're stuck playing a worthless idiot MC, the player should be able to say 'fuck it I don't care about the forced heroine anyway, I'll pursue this more interesting character instead.' But no, most NTR writing is trash and the player is given little to no control over the MC's fate.
The plot twist is that the second heroine is actually the lover of the antagonist who herself was stolen from her previous boyfriend by him and now due to her own weakness has a unjustified disdain towards happy couples.
At this point you just add mechanics that the more 2nd heroine distracts you, the more main heroine gets jealous and the more antagonist poisons her mind. Make it hard yet possible to fend off 2nd heroine advances and discover the antagonist getting closer to main heroine. Hell, you can even have it to where not only does the MC gets taken in by the 2nd heroine but fixes that broken mentality of hers and she truly falls for him or honestly seeks forgiveness from her ex making a bittersweet ending where the MC does lose heroine but he gained a ally instead and let that bleed over into NG+ where 2nd heroine isn't as aggressive, gives hints to the antagonist's plan and even helps you smooth out things with main heroine once done (of course keeping the fact that she and MC had a physical connection secret).
BUT that wouldn't work for this type of game, and honestly I wouldn't know what would while keeping it NTR themed without making MC either brain dead or a total wimp. At least, unless I had a bit of time.
