Hm that's tough. I can think of Summertime Saga as an obvious candidate. There's a also quite a few japanese games that have pregnancy/birth counters (more than I could list right now). It's probably a good idea to just browse through the site a little and look for games that are tagged with the pregnancy tag. Sadly, pregnancy is a tag that's often fumbled in erotic games either through being a lazy one-and-done type of thing or by simply being an ending. And even the games that do have actual pregnancy mechanics often fall flat because they lack depth for that specific mechanic. There still hasn't been a definitive pregnancy game so... gotta keep looking out for it.
