Has Lewdlab in any way ever addressed the reason why the pacing and maybe even the writing have been so off compared to Dreams of Desire?

His character designs are still top of the crop for me, but it's been 1.5 years, 10 updates and we are at the point where the sister is giving the MC a handy now?
Edit: The story is also moving at a snail's pace. There's this drug everybody is talking and raving on about, and the MC has not once used it or at least seen it in action. I was under the impression that it would serve the same purpose as the spell in DoD, but it's just a non factor at this point, we could just as well be distributing cocaine.

Is it lack of inspiration, laziness, or a mammoth project that will take 5+ years to completion?
Don't want to think that the developer of the game that made me a fan of this porn genre is now milking his second creation.

No, its the same patch. Believe me, its some sort of a renaming shenanigan going on when you are saving and naming it. Probably when you rename it you don't do so to make it an extension and thus it remains an unassociated file. Because otherwise thats the exact same patch. I myself downloaded the one from Lewdpatcher and works perfectly.

What matters is that you have a working one. Stick to it. Enjoy.
