I believe there is a scene missing.
Two updates ago the MC acquired the butt plug. As far as I can remember there was no task or scene of him giving Madeline the butt plug. How did she get a hold of it?

It is by far the worst part of the game, it doesn't make much sense at all why it is there in the first place, and would argue it is a poor and lazy excuse to force scenes.
The (repeatable) intimate scenes would be far better if they occur in a natural setting (girls sneak in during the night, escalating scenes during tv or workout or swimming, a kiss here, a touch there, even the events outside could have interesting repeatables but those stopped all together). But nope, nothing there is repeatable.
She was given it in the last iteration. She just didn't do anything with it (providing you bought it of course).

I appreciate the explanation about how deep incest goes because I haven't played the game and was very curious about this aspect. Maybe it's enough to warrant the incest tag.
However, it's not enough to get me to forget how superficial incest was in his previous game. It was literally devoid of all kink, like taking a random German porn movie, adding English subtitles with "oh brother fuck me yeah!" and then selling that as hot incest porn. In the first game this kink was reasonably hot. I hope this one manages something similar by the time it completes but I wouldn't bet on it.

It was given to her that night in the bedroom before the BJ scene. So she already had it inserted by the time she was gardening again. We don't actually get to see her insert it.

Thanks for the explanation. I went back to the BJ scene and sure enough, she is given the gift at the beginning. I was do distracted by the scene that I forgot about the gift.

It is not needed as there is already a key file included in the archive.

It should already be included in the game's main (root) directory, after the game is extracted to its own folder. If not, that's where it needs to be.
