IIRC all you have to do is blackmail Karen into throatjob during Day 12 evening.

I did not do that because Im trying to figure out the hidden ending scene. I was wondering if I need to keep Karen happy and not do that to get the ending however I also know that I need to corupt claudia to have that occur. So I guess that does not matter
