On the other hand, designs for the girls vary from 'this looks reeeeeeeeeeaaaally nice~' to 'would not touch with a 10-foot long cattleprod'...

Do people really question the realism on fictional works with magic and other similar stuff ? It's ok that they can smash boulders with their bare hands, shoot fire and lightning with their thoughts alone and survive beatings from enemies with said powers but big boobas, weird anatomy and not having back pain is so unrealistic.

It's suspension of disbelief.
Smashing boulders with your bare hands is acceptable because the basic action can be understood by the audience. Shooting fire and lightning is also something you'd expect from magic.
However, if Joey from Friends suddenly crushed a boulder with his hand or shot lightning out of his fingers it'd be really strange. Even though you saw Emperor Palpatine do it in Star Wars, on Friends it makes no sense. However, if it were a dream episode or he was acting in a TV show and it was portrayed as special effects, no one would think twice about it and might even laugh.
Comically oversized breasts are very similar. Ludicrous proportions are often seen as comedic. Big hands, oversized heads, giant feet, big ears, etc. All seen as comedy. So there exists this fine line for people between "this looks good to me" and "this looks hilarious/gross to me." The larger they get the more absurd they become as a premise and the harder it is for the audience to take them seriously. But that line can be different for everyone. Me personally, the breasts on the two demon ladies is excessive, but on the goblin it's perfect. That might not be you, but personally I think we should respect that.
Not everyone is going to understand everyone's predilections and kinks, and there's nothing wrong with saying you don't understand someone liking something. Because it's a simple fact.
