The patch is written in a way that follows the storyline so it feels as canon as the original story does. You aren't losing anything by playing with either. It's just whatever your flavor is.

I am currently playing the Steam version without the patch and honestly the story and relationships so far makes sense and kinda explains why MC is like he is, so I don't think the patch is necessary unless you really want to play a story with incest.
