Anybody who has the required skills can do it, if he likes the game. Why it's not leaked at the same time as the game? Perhaps because the author of the patch needs to have the game to make it, and it takes some time. About your last question, it's quite possible that Patreon watches what happens here, and elsewhere.

It's simple. Patreon doesn't accept incest. If you create a game with hidden incest and also create a patch to reveal it, you violate their rules. So the patch has to be created by somebody else.
[Edit] Of course the incestuous scenes are in the patch, the developper has nothing to do with that.

There's no content in the patch, it's just a different script file.

Not that scene, the one where she's sitting on the desk with her panties out and legs opened. I know it's part of the 2nd choice of the 3 because she sits on the desk. But that one frame/picture that the person i quoted posted i cannot seem to find and others as well.
