The dev didn’t classify it as an additional ending, but if you’re into pregnancy content you can get a slightly different ending depending on whether she’s pregnant or not in the twins area. So technically I would refer to it as ending 4a (not pregnant) and ending 4b (pregnant)6 Endings (going by memory)
1st ending (normal)
2nd ending (confront, pick 'she')
3rd ending (confront, pick 'none')
4th ending (twins)
5th ending (triplets)
6th ending milf (fusion)

I’m really glad to see my incest patch made such an impact, it took me months of planning and coding to integrate it into the story so that it made sense and didn’t break immersion. I was disappointed by the only other incest patch that was available, so I decided to create my own. I was worried how people would rate it, so seeing your comment is a reliefI had skipped this game for a while despite it having interesting tags, thinking it would be a rather boring romance story with an inauthentic incest patch that doesn't actually make much sense. Got bored, tried it anyway, and I was very mistaken in my earlier assumptions.
There is no romance, instead the story is rapey as fuck. MC brings up arguments like "If you didnt want me to fuck you, you could have murdered me in my sleep". The LI fears to be fucked to death and agrees to become MCs girlfriend to avoid that fate. Eventually she starts to enjoy it. There are later revelations providing some recontextualization to this rapeyness, but that comes only after. Somehow I found it very hot, and the rapeyness is sufficiently gentle that I could enjoy the game anyway.
The incest patch makes a lot of sense. In fact, it might make the game make more sense that without - it offers an explanation why the LI sticks around etc while rejecting MCs sexual advances.
I think what I enjoyed the most was just the brazeness of the MC telling his sister that he wants her to have his babies essentially out of nowhere (and then making her agree to it).

I found that confusing as well, tried clicking on it 'n such. It's not obvious that it's a reference to a (at that point still unknown) spell you need to learn/activate. (Btw: in the context of a spell book, I think it's more natural to have it such that you're learning a new spell, and not somehow 'activating' one.)You need to activate "pain is pleasure". I assume I will modify the mini-game to make it more intuitive.

One way the spanking screen can be improved I think, is that once that you need the spell to be able to progress to level 2, to display a notice "You need to acquire a level 2 spell to progress" instead.
Perhaps something to consider for inclusion in your mod, @DeftestHawk?
