Did you let the doctor have fun with Ava? Doing that locks her in to not having a dick. There's no real logical reason why, I think the dev just didn't want to make twice the amount of renders for the doctor's office sex scene. You need to choose the "I'm interested in her new body and wants to be her first" option when it comes up, then the "Hand her over to Lucas" option.

Just did a quick skip through a new game to verify, and it's definitely possible to see the scene. The 32+ share point requirement at Lucas' motel is probably way too tight though; the maximum you can get by that point is 33. This was all on the incest/subscribestar version by the way, but it should be the same on the patreon version (I kept the script files from that since I wanted to compare what all was different).
