I don’t think so. I’ve never seen a harem or incest-focused dev create a third path where you can play from an NTR/NTS perspective, there's a lot of people that hate incest (me included) and we never get this option — that just doesn’t happen. So why should NTR games cater to people who don’t like NTR? If they don’t like it, they should just play something else. It’s that simple. The game is literally called 'My Italian Slutwife' — why the hell should there be a third option with an 'alpha MC'? The whole point of the game is NTS/NTR. Sure, we could have some scenes with other women, maybe even some good swinging with the wife encouraging the fun. But a full harem-style path where only the MC gets to fuck and is called an Alpha because of that? Lol, no.
i hate ntr ,but only when the plot or script is too dumb or the male protagonist is only constant humilliated by the li,...i played games where the ntr is unavoidable and i dont have problems till devs put some idiotic excuses to why ,when and how....is easy im an harem fan but when it comes to decide wich of the only one liallowed or any ntr game i prefer the later,dunno why,....maybe the ntr category have the best steamy sex scenes of all? another thing to is when there is only the female protagonist and they put the tag ntr ,is it? if iam the only one cheating why is ntr?
To stay somewhat close to the game the way it is the only way I could imagine the MC gains a bit more power would be if she went into porn or onlyfans and he acted as her manager or something like that or he becomes her pimp and just pimps her out haha
I understand your taste, because mine is pretty similar. Personally, I hate hard NTR and anything involving humiliation. I also don’t enjoy harem games — they’re boring to me. Same goes for incest content, which honestly disgusts me. I’ve never been able to play or engage with that kind of story; I just find it weird. The idea of playing as one guy who sleeps with everyone, including family members, feels so plain and repetitive.
That’s why I prefer swinging, NTS, and non-humiliation NTR — because I want options. I want to choose the LI I care about, but still enjoy seeing scenes involving other characters too. I even remember playing a game like that once, where I managed to avoid the incest content, but still had the MC sleeping with other women while his mom could also have some fun with other guys. Sadly, I never found that game again — and to this day, I haven’t seen another one offering that kind of freedom and variety.

Well, you're right, if you don't like it, you shouldn't play it just to get mad at it later, but we have to admit that the character models in these types of games are much better than in others. I see a lot of complaints about the quality of other games, hahaha.
Or like I said — swinging. Having another female character in the game as the wife’s partner in crime, maybe another hotwife, with the option for the MC to sleep with her too — that would be great and spicy. But a full-on 'alpha route,' as some call it? Wich resumes in the MC fucking everyone and no male being able to rival or compete with him? That’s completely unrealistic for this type of game and would just turn it into another plain, generic harem.
I still remember that game set on an island where everyone supposedly slept with everyone… but of course, our MC — a skinny kid — was allowed to sleep with his mom, sister, and every other woman, while no one else could even look at his family. The game got a NTR tag, but it's because the couples ingame fucked each other before "Alpha" skinny Mc step in. No wonder it got abandoned.

Depends on the game — I don’t really follow harem or incest stuff, so I can’t speak about the models used there. But Medical Special Care, for example, features exactly what you mentioned: three paths — Love Route, NTR, and NTS. You can play as a full-on 'alpha' male MC if that’s your thing, just keep the wife on the faithful/love path while the male MC takes some cheating and netorase options.
The game offers a lot of flexibility, which is why it’s my favorite. And yeah, it’s also a perfect example of what you said — great-looking models (Eva the Wife is the most hottest Milf I ever saw In a game) and solid content.

When I first played the game, I was shocked by Rafaella's nature: she has no shame at all. The only thing I found strange was that at no point did our protagonist get angry or show it in a more emotional way. I think that, deep down, he has a fetish for being a cuckold, because if anyone else saw his wife, with two guys grabbing her like that, they would be very angry.
