After 10 hours of playing... This game is a mess ! Don't get me wrong ! This game have the most content in here. But there is no way to keep playing it !
And it looks like the dev changed a lot of things in the mid way of the game, as reading the walkthrough. it's really different or some of the stuff walkthrough mentioned is completely gone.
And i keep reading people saying "Game breaking bug", And Oh Lord Behold ! i just encountered one just now. Completely stuck or some woman's pictures stuck on the screen permanently, with my save that worth 5 hours of playing.
The walkthrough is outdated... Mostly the walkthrough modders gave up on it, i guess.
Well it was nice knowing it, Hop to the next game !

So many years, game still a fck ton of GB to download, won't do it, still more content reading the comments how people are annoyed over it, or chill about it.
Either way, this is a game that'll be milked until the internet dies, or the creator himself dies from old age

I haven't played in a few updates, but as far as I remember from that point, there was no "better choice", it was just a matter of which of the mafia-exclusive girls you were interested in having scenes with. I think the story is overall the same, aside from the direct mafia-related story stuff, and even then it was largely similar at the time.
Things could have changed since then, though, honestly. All I know is that the way it laid things out was that certain girls might be exclusive (or become locked out) by choosing certain mafias, and it was generally made fairly obvious who those girls would be (i.e. the ones you interacted with in each of the mafia "introduction" quest lines, and a couple others that they telegraphed within said quest lines).
That said, I do think any game that has this much content/takes this long to go through things and has path-exclusive choices like that, which lock you out of seeing content with one (or more) girls by choosing to see content with another, and not having an option to see content with all of them, is always disappointing. Because nobody is going to waste the time to keep up three separate saves to play through all three mafia's content.

Characters that aren't one shot characters from what I have seen Mrs. Jordon and Caroline. Otherwise it is just a bunch of random one shot characters that take place within certain events.Pregnancy? Who can be impregnated in this?

Thanks for the explanations, I remember a little more - Girls (families) were each for a DIFFERENT faction = for example Stepmother (for the Japanese), older sister for Bees, etc. The choice greatly influenced the relationships in the family And a girl from another faction could die, for example = if we chose BEES then the girl of the Gentlemen (who taught the hero to shoot) died, or vice versa... This was a VERY long time ago and therefore I do not know how much this is implemented....
