Every H-scene in this game has sort of a "second round" (like, Blowjob->Titjob, Vaginal Missionary->Anal Doggy), which are locked behind the paywall in free version, while the full version is, well, full and has all content unlocked. Plus there's also the "dream" scenes which let you non-canonically shag dead/Naruto characters. And there are also sex-training scenes, with about half of them also being paywalled.
Is that avoidable or is that a common theme in this game? Concerning the "Horror" tag - there's some very graphic imagery and bodyhorror moments featured in this game. As for the three special letters? Most of the major heroines in this game (Hinata, Sakura, Ino) have husbands, whom they're slowly estranged from as their stories and addiction to MC's dick progresses, so it's kind of obvious that it's a main part of the game.
So I'm assuming based on your answer it's not avoidable. I mentioned those three letters or rather the hard version, because fans of it enjoy the schadenfruede i.e a deceased Naruto being a trophy. As for me, I don't.
