So when I'm testing some scenes IG, I'm almost everytime confronted by choices like "Scream" or "Move away" without any way to engage a sexual encounter, like the scene with a friend and two thug which always end in "Scream". I've tested every type of character,to bitchy to nice, sexual to down to earth, all to no avail. Any tip please ?

There is no non-con content in this game. Stop trying, you can only fuck the guy that saves you.

Anyone else noticed that in recent updates when it comes to custom npcs, sure, they load all the ones you make but then still create a bunch of shitty randos too? It used to not do that, and im not really sure why it is now.

I'm pretty sure if you have enough custom NPCs, you'll fill out the roster. You have to make sure you have specified all the 'special' NPCs that you can. Dance students and the dance instructor, for example, are generated randomly the first time you take a dance class if you don't have defined ones.

Whenever you want a playthrough with specific kinds of NPCs, you can set the NPC limit to e.g. 20, and then create 20 "blank" people, with their templates just specifying the things you want/don't want (the stats you leave blank will be randomized, so you don't have to deal with an army of clones). As gobbet said, some special NPCs still get created, and those will push out some of your created NPCs, but the rest will be just as you like them.

I do have all the special positions filled. I have 27 custom npcs, dance instructor/judge/, cooking instructor and students, gym jerk, boss, 3 lowlives, repairmen, porn actors and 12 with no special title. They all appear in game. However, along with them, 15 more, including more low lives, cooking students and everything else are in-game. I dont know why. How do you set an npc limit?

Now that I took another look, it says in the options you can only set the limit for additional NPCs, not the general number. Although if I get this correctly, you could set it lower than your number of custom NPC and it should at least minimize the odds of running into any people you didn't want in the game

Maybe. I've had it set to zero. Though afaik the npc limit for randoms was 12 to begin with so how i get 42 in a game is beyond me.
