You need to go back to the forest where you first found the ghoul, same spot. You go through the last cutscene and you will have the vampire girl unlocked as an optional party member.

I am quite sure you ask a clerk at the guild to raise your adventurer rank once you have done enough quests. From then on you can unlock other plot-related quests. Dunno about Iris missions since I don't do them.

That looks to be an equipment meant for a follower, which should be PM. Try to equip this to her when she is in your party.
