If i remember correctly you need either 150 or 50 fame points which you can get from completing quests or sometimes beating strong enemies and once you get those fame points you'll talk to the guild manager at the main city and choose promotion.
Once you complete all her relationship scenes whenever you leave the city or enter the sewer the game will ask if you want to bring her along as a follower.

For advancing to rank 2 , there is a error in the current version . You might have to install the patch version to rectify this issue .
Have you tried going to the Alchemy guild? It's on the bottom left of the desert town. Once you clear the quests there you should be able to progress with her.
partner with her and go to wildness city(i dont remember the name) then go to the library(the first building on the left at where you enter the city)
