Despite her parents I don't think a belly button piercing is out of the question for Holly. We'll have to see, but if Holly is being heavily influenced by Lena and Ivy AND if Lena has a poor relationship with her parents I could easily see Holly entering her "rebellious phase" at 23, but whatever. Does she have in other scenes in both previous and new chapter's or it is in this scene only? If it is only in this scene than that is an oversight on EK's end.

I knew I remembered it being a part of her look, but I could never find evidence of it in the game after that one update, literally went back and dug up an older version of the game to find it. As far as I know it's not possible to get Alison to have that look now. This was back when all the female love interests had three outfits depending on how much lust Ian had been showing towards them. I remember Cindy, Cherry, and Emma also having permanent wardrobe changes depending on how many times Ian had, had sex with them.
I do wonder if it will be re-implemented later, either as a continuation of Alison's bimbo route or it is just something she just decides to get, because she wants to.

It's changed a couple of times.
Around chapter 3-4 Eva said it would be about 20 chapters, then around chapters 10-11 it dropped to 16, then the patrons over at Patreon threw a hissy fit that Eva said 20 when it wasn't really clear how much story there would be to tell in the early stages of the game so Eva said there would be 20 again to appease the patrons.
So to answer your questions somewhere between 12 chapters and a never ending story.

There is a free pdf file attached to his patreon link in the post you quoted. You just have to read the pdf and make your desired path/s accordingly as described in the walkthrough as the game is pretty big and the path/s can be easily missed. It's just for assistance. If you don't wanna used it you can used JDMOD, link is in OP.

Idk how it's taken me this long but I noticed a tiny detail about Axel's Instagram (Peoplegram... whatever) display picture. It's identical to one of Lena's poses in her first photoshoot with Danny in chapter 1. Maybe Lena used the same pose when she modeled for Axel and he still has the pictures? Idk if it's been mentioned in this thread or on Patreon before, but is this just coincidental or is this a subtle way to show just how embarrassingly hung up he is?

I can see the similarity, but also the subtle difference, mainly in the color, but I don't think it's Lena, not really her, I guess if it were her, Lena would have realized it a while ago, what I think is that it is Lena's drawing, but not Lena in the game's fiction, I think Eva Kiss only recycled that Lena drawing for Axel's account, since it wasn't something important, just changing a little the color, I would do the same.
kekw well whatever hopefully we just get good character arcs and closure on some of them I trust them to finish it up nicely

Ian is 5'9; according to the MMA height average, he's probably in the lightweight or welterweight division. The first opponent that Ian fought looked like a middleweight player.

I couldn't find too many that were drastically different. Most of it was clothes that they already wear in this version of the game, just in slightly different scenes. But here are some of the bigger changes I noticed.
Alison with the belly piercing, but I think her dress was a little darker here, also her nipple was slipping out. I don't recall her nipple slipping out in the current version.
The biggest change, that I think any long time player remembers was Billy used to be black.
Lena's old sporty attire. She wears a sports bra now instead of a half zip.
This is a scene that now takes place in the mall. So not a character change, but the background change was immediately noticeable to me. Though it is kind of funny to imagine the three of them just walking around down town in their underwear.
Bad Jess used to have a red top, like her good counterpart. Now it's leopard print.
Stan's old design. Somehow he got a glow-up despite wearing worse clothes now.
The signs that Stan was going to be an incel all along.

So whats everyones ranking of the guys?
I sorta exclude Ian since he the MC and his personality and performance varies from loser to winner, cuck to chad etc., he is winning by default since you don't experience most sex scenes without him.
After that, best is Axel, he has a oppressive presence and its good that he is used sparingly. His sex scenes are prime domination and great for ntr and corruption. A bit of a shame that the redemption/bro ith Ian arc as dropped, but once Cindy and possibly Cherry get rolling, it will be better.
Jeremy is annoying for sure, but his range and the variety with how many women he can end up (or not) make him a great, if not needed secondary male character. You get bro sharing, competition, cheating, all necessary elements for the porn aspecs of the game. The Lena-Louise-Ivy angle can be fun to mingle with, but the Alison/Emma sharing also has its peaks.
Mike is currently a bit stale since the temptation aspect of him and Lena have run its course. His gf has to be adressed and damn if it isn't Gillian, that ranks the drama and interaction with other characters up.
Seymour dropped the ball after chapter 9, the game isn't really benefiting from an antagonist like him. The Lena content is a one-way road, he has much less range than Axel. Maybe a colaboration with Agnes and Agnes, or throwing in another girl?
Robert gets a few points just through quantity, the sex scenes are mostly fine and it leading to a bit of rivalry with Ian gives it some meaning. Curious how the Holly content works out, it seems Eva doesn't simply drop him.
Stan is awful on the romantic route, his saving grace is the pay pet route with exhibitionist stalkfap Lena. Exploiting him on that route can escalate even further and its a good take on the hot girl ugly dude dynamic.
Perry currently has low content, but wingmaning your less fortunate friend and granting him a girl or two over the course of the game can be rewarding, there is also the teasing angle with him and Lena.
Marcel and his scene came out of nowhere. I prefered the odd pairing with Holly, it is a much spicier chpice for a corrupted Holly than Lena fucking him just because of BBC.
Jack was a random cameo, I don't get dialogue hinting that the two of them may meet again and develop their "relationship". I'd rather have him fuck Ivy.
Mark is the blandest generic love option either, he has nothing to say, no personality, and yet Lena and Holly can develop a relationship with him going further than a one-night-stand. I rage if that bland piece of wood turns out to be Gillians boyfriend.
