None of that happened in my playthrough. Ian in my playthrough has only definitely been with Gillian and Lena. None other mentioned, but more are possible in the past. Lena has only slept with Ian. No others. Holly, poor thing, hasn't slept with anyone at all.

The only way a harem could happen is if both Ian and Lena share all love interests specifically Lena both male and female. This have more chance to be as a power trip when using drugs or Eva make some sort of fake ending/breaking the fourth wall because there's no way that a harem could work. You serious? Ivy is The Corruptor. She (as is always the case with such characters) has significantly more experience than either of them, probably more than all of them combined. She takes control of Lena's and Holly's pleasure from the get go, something that comes only with experience. She teaches both of them pleasures of the flesh and how to better achieve them, even states that inexperienced mouth can't make her cum. Again, that's something only very experienced person could do. The only difference is that Ivy values herself and thus is always in control: she wouldn't meet with someone she deems not cool enough, if it isn't for corruption purposes, she wouldn't perfom certain acts without being generously compensated and so on.
Of course, that take completely omits that "Holly becomes more outgoing, sexier and confident" in this particular scenario actually means/includes "Holly starts hooking up with a bunch of dudes she barely knows". Which makes the escalation to gloryhole far less of a poor development.
Considering Jeremy who gets to fuck Lena is still head over heels for Ivy (and how Ian similarly is pretty smitten with Ivy even when he's in relationship with Lena from early on) it does say something about how Lena/Ivy are positioned relative to each other. And yes, getting it on with Ivy is clearly something those two would consider an accomplishment, they say as much.
Maybe it didn't happen in your playthrough, but it can happen in others. Which means within in this game, Lena, Ian and Holly all have the potential to be more promiscuous than Ivy. It may very well be, that outside the game Ivy is fucking around, left , right and centre, but so far the evidence is all circumstantial. Whereas, with these three we can actually see it.
You're failing to understand what "canon" means. Canon is true in all playthroughs. Your results are what you got based on your choices, and you can talk about that from that perspective. Your results are not canonical to the game as a whole, just in your specific playthrough. None of these characters (except Ivy and Axel, and some of the support players) have a definite personality because they are all dependent on Ian and Lena's choices (really, the player's choices, right?). This is what makes ORS so amazing. Everything is so very variable and interactive. I've never played a game that was so different based on choices.
No, you're failing to understand what I'm talking about. I suggested that in SOME circumstances Ivy was not the most promiscuous person in the game, as is generally assumed. Obviously, on some playthroughs that is not the case, but on a lot it definitely is. But people still act like Lena, Ian and Holly are not fucking around more than she is. Ivy might give the distinct impression that she's very promiscous, but we see little direct evidence of it within the game. So we don't really know if it's happening or not, or at least how frequently. But in the case of those three characters, we see everything they do, so we know for a fact, exactly how promiscuous they can be.
