It's pretty subtle at times, and again usually most noticeable during sex scene because he'll do thing differently depending on the fit tag or not.

For Lena to participate in the 3some, she just has to not be dating Ian.

For the second screenshot the relationship value may be maxed but ian_lena_love is not max (2). That is one of the conditions to unlock that option. Alternatively ian_lena_over = True is another condition. To set ian_lena_love at 2, you have to tell Jeremy at the club in chapter 7 that you're falling for Lena.
