PilotLara seems there's a minor error in the new update, in the scene where Lena takes a picture at the pool:

                elif lena_bikini == 3:
                    show v14_pool_selfie_bg3

These image variants aren't defined anywhere in the game's code, so the call silently fails. As a result, Lena always seems to take a topless picture, even if internally the game recognizes/considers she's clothed.

Ah I think the problem lies elsewhere, as there is no need to define these images (they are not composites, the game calls them straight from the image folder and they do exist over there). I think you're on the axel route, yes?
I saw that one had the base image at the wrong place. Is this the fix to your issue?

Scene on page 47 - "Mike calls his girlfriend. Lena & Mike"
You need to choose an answer for question:
Did Lena like the makeup style Zarina showed her? - Yes, she loved it
The next choice is unimportant.
And then in the scene after Mike says
"Yeah, sorry, I..."
An animation plays where Lena's tattoos appear and disappear on her left arm and left leg, even though they shouldn't be there.
