I'm catching some of my other paths up before I continue my main save. I was wondering does Lena have to not be on the Ian path for this choice. As I did a speed run with cheat mode and still couldn't get it, so it wasn't due to a lack of attribute / skill points.

Because as far as I can tell, EK is very anti-swinger. The allison-ian-jeremy thing is iffy at best, and in all other cases it's clear her tastes require a 'winner' and a 'loser'. (except when it comes to mffs, then she's very democratic) Which is a damn shame, what I wouldnt give to see Cindy get doubled up on by Ian and Axel.
Coming to think of it, you have a point actually. Even the MFM with Dave in GGGB, where Ashley is very much in love, shows him being the beta male vs. Jack or any partner she chooses for it.
