Normal LenaxHolly romance requires Lena to not be dating Ian (this includes being fuckbudies) and you need Ian and Holly to not be dating each other.
Ian and Lena can have dated and fucked in that playthrough but then you need to find a way to make them break up (e.g have Ian and Robert fight and then have Lena be pissed about that, have Ian be upset at Lena's stalkfap content, + probably some other ways I can't remember right now).
Ivy has been potentially flirting with Ian for a while, if you managed to impress her with earlier game choices. So yes, the idea that there'd be Ian/Ivy scene in addition to Lena/Ivy and Lena/Ian/Ivy is a possibility.
Of course, it is another story if Eva Kiss will ever give up on her vice that is edging SerHawkes

From IanxCindy love path perspective It looks very difficult at this point. Ian already dump Lena for Cindy. Maybe it could happen in LenaxAxel but that would be another Cherry 2.0, buuuut as I said before if Stan can F Lena, anything is possible in this damn game.

She did the same with Louise. I hope we can prove Ivy wrong, I am also thinking of making a solo run just for Ivy.
