You don't even have to reject holly at the very start if you don't want to. Kiss her with Ian and dont sleep with her when they are on their trip. You can even let Lena & Holly kiss if you want.
The final decision for the trinity throuple path is when Ian & Lena go to perry's beach house, where they talk about Holly. Ian there has choices while talking to Lena in their room and one of the choice there is something like "It's good you told her nothing will happen" something along these lines, which closes the holly trinity route. And after that the open relationship route actually gets momentum. Check this message out and you'll know what the future holds for that route:

I have one route where Lena is a pure bitch. She bullies Stan, steals from Molly and Ed, hates the homeless, kicks the dog, hates her mother and also hates Cindy's guts. She also has an affair with Seymour and Axel while in a relationship with Ian. And she successfully hides it from Ian. Though Ian is no saint either. He is cheating with everyone he can get his grubby paws on. I call this the evil Ian and Lena playthrough, with Ian focusing on charisma stat and Lena focusing on wits stat.

Ivy flirts with a lot of men, but that doesn't mean she wants them exclusively. That is some Stan level take NGL, friendly flirting should not be misinterprated as love or even liking that person differently.

I like the chemistry class but that word scramble game is pure BS if you are on a rerun. ORS' mini-games are child's play.

I'm referring to the current Ivy and my understanding of her character in most of my playthroughs.

For me evil Lena based on Wits is the canon one. Although I don't pair her with Axel as it kind of makes her weak-willed (Axel de facto manipulates her) while with Seymour it is possible to retain independence.
