Not necessarily. If Lena fucks Axel in chapter 9, but ignores him in chapter 10 and 11, she won't fuck him at Blazers and Jeremy can't tell Ian about it.
In general, Jeremy tells Ian because the Blazer guys (Ivy, Mike, Marcel, Mark, Jeremy) tell each other who they fuck.

Is there a way for Lena to cheat WITH Jeremy while in a relationship with Ian? All variations I tried Lena ends up not dating Ian if she did something with Jeremy sexually.

There isn't. You are spot on that Lena won't cheat on Ian with Jeremy, because they are friends, which she considers different if she cheats on him with someone unrelated to him.
