Ah I think the problem lies elsewhere, as there is no need to define these images (they are not composites, the game calls them straight from the image folder and they do exist over there). I think you're on the axel route, yes?
I saw that one had the base image at the wrong place. Is this the fix to your issue?

Hmm, no, this isn't limited to Axel route, it happens also if Lena chooses option to take a selfie, and likely on all other routes, seeing how the relevant block is copy/pasted in all branches:

Effectively, game only shows the last variant which is Lena topless. All others (the ones with _bg extension, including the _smoke one) are not defined, so they don't have any effect (and if they did, if the image is not composite then with how this block is structured, they'd likely get obstructed by the final topless image anyway)

The game is amazing, the whole plot is very well put together.
It's so obvious that the person making the game wants to please whoever is paying.
I don't know why there's so much crying over one "gay" scene.
Are they just two men having fun and being happy?
If you're comfortable with your sexuality, this shouldn't be a problem. If you really can't handle it, I suggest you seek therapy or just go to another game!

IIRC no, it's one of possibilities but it's not required. Generally, Lena's option open up considerably if she has fucked Seymour, since this clears her potential _pure flag in relationship with Ian or in Ian/Holly trinity. But she also can simply opted to keep cheating on Ian if she did before and Ian let it slide.

I don't know why some of you are shocked by gay content in the game and it's avoidable. It's not the first game that have it and certainly not every porn game dev include warnings nor they make everything avoidable like some Devs do. I would expect this reaction from someone who just discovered porn games not regular players. Did GGGB had reactions like this with some of the content? That game didn't had any warnings too and the game have one of the most hardcore kinks. When I first played GGGB I was disgusted by Jasmine and it was more by her sexual harassment of Ashley then the fact that she is a transgender. Thankfully, you can avoid her scenes. Though, many players weren't bothered by her ofc.

Again, what did you think you would see when you chose to peek on a man you knew was gay (something you have acknowledged game has told you earlier) while in a room with another guy you have sent there specifically, with Lena blushing like crazy when she suggested it? In a porn game, where "peek" options basically always provide the player with scenes of other people having sex?

The icons on this option suggest it should be gated behind Lena's relationship with Danny and charisma, but it doesn't have any actual requirements. Should probably copy the ones from earlier instance where the whole mini-arc is established:
