Milked? Not if the Dev (singular) update on a regular basis, and contents are delivered as promised. Nothing was "withhold" for next update without being disclosed. If he is late, he says so. No delay longer than a couple weeks. By the way, how much did he milk you?
NTR? That's just because confused players want it their. The MC does all of the cucking, and some sharing options. Raped by other than the MC was perceived as NTR. Also, the MC clone himself would be NYR as well.
And has a full time job separate from creating PQ.
They asked for more goblins, and got them. They asked for more giant content, and got it (and are still asking for Siv). They asked for more Beth, Gwynneth, Maghda, and Vixenatrix content, and got it for all of them. He mentioned a cow-girl and they went YAAASSSSS!!! so Annabelle was added.
It's refreshing to have a dev who listens to what supporters are asking for and tries to give it to them while maintaining their own vision.

Depends on what you mean by that. The game has always had some form of NTR content, but it was never the focus.
