This reminds me of something that I noticed: Once you unlock Cream Machine, if you fuck a girl 3 times her eyeliner starts running and on the 5th time she does her impression of a dying cockroach. Pretty cool, and I like it... but now that you mentioned sex minigames, that got me to thinking...
Before we even start talking about minigames lets not forget the importance of stats, yes? You already have a Sex Skill for the MC, and that should be enough. Actually, change that - let's add a Stamina Skill (which is the average of Sex & Fitness, as opposed to something directly trained) and let that take the place of Cream Machine. Say 1 organsm per sex session for every 20 points (so once you get to 100 Stamina, having mastered 100 Sex and 100 Fitness, you can go 5 times in a row).
Then on the girl's side getting her off could just be a factor of how good your stats are. OK, maybe you can insert some kind of minigame in here, but stats should still be the driving factor of it - like how many chances you get, how big the target is, how fast things move, etc. For each pass (determined by MC Stamina) you get so long to try to get a girl off, and if you do it fast enough you can do it more than once.
Example: Stamina of 80, so MC can get off 4 times. Each pass for the MC is a 30 second minigame where he's got to hit the moving G-Spot or something three times to make the girl orgasm. If he does it the game doesn't end, though - it keeps on going, with his own orgasms just counting off the timer (Stamina 80, 4 passes, so 2 minutes total minigame time - so how many times can the player hit the G-Spot in 2 minutes of minigame? 3? 6? 12? Who knows?).
Then keep the whole running eyeliner and ahegao like is now - 3 orgasms for the heroin chic look, 5 for sex spaz attack, and who knows if you can get to 10 or something maybe she'll gain some kind of trait or perk (lower pay, boy-girl shoots to 100%, or the Nympho trait - could be random).
Next, for flavor, this opens the door to new sex traits for girls as well. For example a girl with a Super Tight trait would shave 10 seconds per pass off the MC's timer, giving him less time between blowing his loads. However such a girl would also have a 50% chance to count as double orgasm on the path the ahegao on beyond (meaning she might bug out in just 3 or 4 orgasms instead of 5). A girl with Loose Pussy would be the reverse - the MC can last longer (+10 seconds per pass) but she'd also be harder to drive wild (50% per orgasm to not advance towards ahegao). A girl with Oral Fetish might not lose any happiness if she only does blowjobs during a Boy-Girl scene. And, unrelated but while we're at it, might as well throw in Fertile and Infertile traits towards girls to (you guessed it) increase or decrease their pregnancy chances.
And finally, to give all this some kind of meaning other than pointless wank-mechanics, have all of this tie into the final quality of the video. The more times everyone gets off the higher the rating, and bonus points for reaching ahegao milestones along the way. Because if the MC can shoot his load five times and make a girl's eyeballs pop out of her head in the process, you bet you're ass that going to be a video that people are going to watch more often.
Final note: "Cream Machine" perk can be replaced by +40 bonus to Stamina Skill, letting it potentially go all the way to 140.
