As WWG's #2 fan and a top-tier patron, I must say that I am deeply disappointed. I don't even care about the money I've contributed, I'm lucky to earn enough that it's insignificant to me. But I'm wracking my brain to figure out what kind of medical issue could prevent someone from writing dialogue for so many months. A problem with sitting? A problem with using his hands? 
He's mentioned surgery and lack of energy... maybe he had a cancerous growth removed, and the chemotherapy is kicking his ass? IF it's something like that, well, that's horrible luck. In addition to physically suffering, people probably wouldn't believe him if he shared those details online, so why would he bother?
At this point, so many years into working on this game, it's hard to imagine he'd WANT to leave it unfinished. His last communication seems to be in good faith, with his plan to pause payments until the update. If he sticks to that, maybe we'll see the update in a few months? I guess the worst case scenario is he never gets better, and can't ever finish the game.
