I recall there being no difference between anal and vaginal. It's pretty much implied that she's been had in both ways before by other dragons ("...raped from the front, raped from the back, raped upside down, hanging..."), and both of the options lead to the same outcome. Why she thinks anal might be different, I don't know. Wishful thinking, or maybe the back channel is a little better in terms of divine disapproval, but she's basically down to her last 'divine connection point' and such a distinction no longer matters. IDK, it's not really spelled out in any detail.

The 'bad' dragon severance event takes place as soon as you rape her after capture. The 'good' dragon severance event requires you to let Heliose talk to her and initiate the threesome scene, after which she'll ask you to have sex with her, which, if you agree, will result in severance.

She says she could feel the disconnect happening immediately, and the dragon indeed feels her being 'burned out' in the 'bad' severance event, regardless of hole used. Might of course be that getting a dragon baby is extra bad on top of that, but how does she know? Are there Maelys-nagas running around somewhere we don't get to visit?

Probably not. Seems like the birth of nagas (though aren't they more lamias ? They seem more land snake based rather than water snakes... whatever) is a well known fact among elves, so likely she managed to find a way to terminate potential offsprings from her ordeals ? That or the other elves (likely Val or Heloïse) helped her. After all, they know each other and were in the same realm.

I doubt it's that intense. Probably more a very special connection base on dragon's nature and powers ? Messing with time itself would seem a bit too huge of a thing, and toying with time travel in storytelling often ends up messy...
