Unless they added something else in the meanwhile, the only difference is the ending text.

How does it change the text? Like a lot of decisions in the game, it really gives me very little info when it asks me to choose. Instinctively I feel like taking absolute power for yourself is the bad move, but on the other hand the story sort of frames him as an Aragorn figure that is the "rightful ruler" or whatever.

I've found (not the shovel) but, it's north of the blood temple under the palm tree, almost in the middle (at 11on a clock)

To get Merchants go Slink and around Farm Mom house is Blue haired guy.
To get Demons guards you have to finish Succubus quest about rescue her kind from Inquisitor and you should find demon girl in conference hall. The demon guards are bugged, Kal said he will probably fix them in new update. For now if you want demon guards use this
