People who get mad at games having so-called NTR scenes - or even a female protagonist - are those who, for some weird reasons, identify themselves with the fictional pixel protagonist of the game they are playing 
. Consequently, these individuals only play and tolerate games where what happens to the protagonist exactly matches what they would like to happen to themselves in their real life. This because their real life is, on the contrary, very unsatisfactory and not gratifying, for it’s very different from what they dream (for example being a successful alpha male who have exclusive sex with all the hottest girls/women he meets).
Rational people, who are satisfied of their life and don’t have a very low self-esteem, instead, simply play the game like an external observer or a “deus-ex-machina”, who can change the course of a story, just for entertaining purposes. It's like watching a movie or reading a book, with the difference that a game usually gives the player the possibility to influence the plot of the story. Consequently, they know that what happens in the game has no relationship whatsoever with their real life. They don’t need to identify themselves with a fictional, unreal character to delude themselves that their life doesn’t suck.
In conclusion, it's a shame that, for the mental issues of some individuals, every game which is only suspected to have NTR - or some other “undesired” meaningless tags - is subjected to such intolerance and verbal abuse, which greatly limit the developers’ freedom of expression in their creations.
