Yes, turning into a girl is no issue. I know it is possible to have her turn MC into a futa, I have a save of that, but I could not replicate this and I cannot figure out what are the variables that allow that.

Her happiness must at least 5 more than her anger and you can't be in chastity that she isn't authorized to remove (so the mall trial, basically). You have to get the first promotion/be in phase 2. Pick "Talk about being penetrated by a dildo under Ms. Antlers' supervision.", "Anal was nice... but you want a vagina.", "You like the idea of a vagina... but could you keep your penis as well?"
I think you only get one shot so make sure that she's in a good mood before you bring it up.
EDIT: My mistake, I misread the code. You don't have to be in phase 2, or even out of probation. You just have to show up to work erect twice to get the scene that triggers this conversation, and then the Doc kind of busts it out on you immediately when you walk into your therapy session. Sooo you can get it by the second session if you talk to her about clothes a few times and emphasize that you prefer femininity. That sort of thing increases her affection. Here's a save at that point where she's happy enough.

So is there a way to go outside without panties? Cause I kept trying to go to the therapist and going commando there but after talking about it I still can't go commando. It should be unlockable through the therapist. Haven't tried it but here's what the dev said last year:
Dunno. There's a way to unlock the body shop through the therapist, but I think that involves swapping the MC's genitals, which doesn't seem like it'd work for you.
The MC can temporarily increase their size using Twig's drugs, but I think those only become available after the pegging "promotion" event as well.
There might be an alternative to the flashing by talking to Kurt after cleaning his flat on a Sunday.
I know there was an alternate route for something to do with the retention manager added there, but don't remember the specifics and don't have the file to hand to check.
Here's the thing, I already did that but the only option I had was flirting with her and it did nothing else besides make her say she's glad I was exploring my desires more. No willpower check whatsoever.

Well, I'm sure you've figured it out by now but you have to tell her about losing your anal virginity to a penis (or a strap on if you set the fetish flag that they are equivalent to true) first. Talking about your lack of underwear is exciting for Dr. Tra...er Synthea but the game wants the MC's first time to be somewhat special/she doesn't want to take that from you so you get the broadening your horizons comment until she knows someone else has been in there.
I think those are going to be for the replay feature in the bedroom which uses your current appearance, at least when the stacked image actually gets implemented. The only ways to open up the full bodyshop are the orgy and the office party. So I guess you've already found the fastest strat: miss a game, choose Luna for punishment.
Alternatively you might be able to meet Dr. Twig pretty early, but I think that that the flag that lets you work for him is the physical in stage 3 so it might be pointless. If you have forced medication on, OD on pink pills, and then stop taking your meds you can claim that the pills aren't working anymore and Antlers will send you down to R&D get checked out, or if you have forced chastity on and become a hermaphrodite with Dr. Synthea you can get hauled down to get fitted for a Demon Breaker 9001.
