You have to disable the "involuntarily/forced medical operation" tag in the optional fetishes page. Forgot the exact name, but basically this.
If you want to avoid the CEO doing that altogether, you have to disable the tag on a save before the event. Even just before works.

Okay, let’s be real for a moment, this year has been a lot. It feels like the world keeps hitting the shuffle button on "worst timeline" scenarios, and some days it’s hard not to just wrap yourself in a blanket and hide. Especially (but not limited to) for queer folks, trans folks, everyone under the rainbow spectrum or adjacent, things can feel quite shitty.
But we’re still here. You are still here.
Pride isn’t just glitter and parties (though those are great, I'd love to visit pride one day [I did the year prior], but my cat decided that last Thursday was the right day to get hurt really bad, so visit at the vet and cat sitting over the weekend).
It’s a reminder that some of this started with a riot and a brick. Some of us come from a history of people who refused to be erased, who fought back even when the odds were stacked against them. That legacy lives on in us (Not saying you should riot and arm yourself with bricks, but I am also not saying you shouldn't :3).
Fun fact, for anyone interested (you read this far, so that your own fault :3), when I started this game waaaay back in 2017, I thought I was just someone with a certain fetish, that I liked to "dress". I grew up in a world where Trans people where either a punchline or straight up villains (Looking at you Silent of the Lambs and Ace Ventura... Yes, I am that old).
I worked on this game (with the help of my amazing team) for over 5 years, before I realized that I might be Trans. Now it's my 5th month on HRT. I met so many amazing people in all of these times all with their own quirks and problems. And I wish them all the very best.

Yeah, it changes their gender but not their genitals.
Rewriting every scene to allow for customizable genitals for every major character would be a much larger endeavour. I believe the devs' stance on it is "only if someone pays us a significant amount of money for that specific addition", otherwise they think their energy is better spent elsewhere.
