The threesome is a repeat of their enslavement scene. If you either are in the living room you can ask for threesome, although it is really a twosome with one looking.

As someone previously said, you need to buy (from the store app) and use fertility drug prior to the deed, BUT you also need to 'talk' to the girl prior and make sure that she isn't on birth control. I had the same issue...
