And then they topped that scene with the Amigdala-Worm boss in the Underground Cave.
Well, I wasn't going to list off all of em, especially cause again, each person probably has a different level of tolerance, but yeah.
Also on your list I'm not sure if you counted in your 9 when the parasite explodes out of Lilias face when you get 5 stacks.

But speaking of non bloody but body horror birthing
You can also see the skin pealing away at the final game over to give way to the otherworldly womb.

To be fair, your mileage might vary depending on where you draw the line for guro. If it has to be like bloody with dismemberment or stuff like that, then yeah, there is only some of that in here. Thou I would say still more than 3-4. However personally I would also consider grotesque body horror even if not bloody in that category. For example the ending for the Leech boss where Lilias body is so full of leeches that you can see them through her skin, I would still count it under the same category.
Guro/ gore means dismemberment, visible flesh etc.
Your snail example would, if anything, fall under ryona, though body horror describes it that much better as a tag

Anyone played the first game recently and can refresh me on the Lore/ story summary/ canon ending of the first game? I've played it a long time ago and I remember the final boss being a priest that transforms into a monster (there is not much story in the first game right?) Is the protagonist/ events of the first game referenced in this game?

FFS, why do you even need to know lore? There is no deep lore, only a few easter eggs.
It's a hentai game, not a Dark Souls.

There is not much lore besides what you deduce by playing, and whether or not you accept the OVAs as canon. You can, however, use the lore of the second game to fill in the gaps.
Basically, Rabiane is sent by the Sisterhood of Purification to cleanse the evil plaguing the land of Kessaria -the Abomination Outbreak. The Otherworlder sits atop the Cathedral waiting, and she has to make her way through different locations filled with abomination broodlings. In contrast with Arcezon, Kessaria has already been fully devastated by the outbreak, so Rabiane's mission is one of extermination and dealing with the source of the problem before it spreads further.

The final fight has Rabiane having sex with a slug the size of a big dog to enter the domain of the Otherworlder and slay its true form. If she fails, Rabiane becomes a Vessel/Mother of the Otherworlder we call "The Observer" (because it looks like a giant eyeball). If she wins and she is not killed in the Pig Farm or the Laboratory... we don't know, the game ends right there. People have speculated that Rabiane is trapped in the Cathedral in the case of winning, or maybe she returned to the Sisterhood and will make a comeback to support Leila.

The "guro" term is often mistaken by Western audiences to mean "gore". Since in Japanese, guro stand for "grotesqueness", which mean malformed, unnatural, or horrific. It doesn't really necessary involved with visible flesh.
And, to be fair, it doesn't really need a lot of gore to have gore tag (if it's exist), one scene is enough. Like if you see a game with pregnant tag, it doesn't need all or almost all women in game get pregnant.

Because some people are interested in the background of the game and just does not want a quick FAP? Why does the game have story anyway if ALL it needs is just H scenes. The developer put a story in the game so that the game has BACKGROUND and the H scenes are more immersive/ has more impact. If you are just going to disparage another user for asking for lore then just don't bother replying and ignore my comment. This game clearly is not a NUKIGE (quick fap game) it has horror story background and "interesting" lore.

I mean, sure, in a way, thou Ryona is used even for stuff where there is no physical harm but simply humiliation or purely sexual rape without any violence otherwise, while this game goes into some pretty extreme horror and body horror scenarios that can be as unsettling as guro, even without the gore itself being present. That's kinda my point.

They are not referenced directly in this game yet thou there is hope for that in a possible future dlc.
The final boss of the first game is an eldritch being much like in this game as far as I'm aware, called the observer. The story itself probably goes rather similar thou as this game. In a way this game has a lot of parallels with the first. A number of priests came into contact with an eldritch god, became corrupted and went mad and started expressing their madness in different ways. Corrupting populace with parasites, trying to expand their minds, mixing humans and monsters. Standard lovecraftian cult activities that is either aimed at pleasing their new god or trying to ascend to it's level or simply just ravings of a madman.
Rabiane was sent by some holy organization to investigate and deal with whatever is going on. Considering there is no dialogue in the first game, we can only really infer from the 2nd game and context clues that the Observer might have had a similar goal to the Father, to have a human female host that is a suitable vessel for him to procreate with / reincarnate from.
Now at the end of the first games good ending we do se the observer be banished and Rabiane leaving the place, however one particular thing of note is that in the final fight Rabiane is put in a haze and can't help but embrace and copulate with a slug that could be the core of the Observer, the same type of slug does appear in the 2nd game after the Father gets blasted with holy energy so it is possible that it is a sort of core or true form for these eldritch gods. Either way. Do to this fact it is questionable even if Rabiane banishes the observer, she might already be impregnated. That however is unconfirmed.
Being as these games are inspired by Lovecraftian horror, a lot of things are foggy and left unanswered completely, as the genre is in large part draws it's horror from the fear of the unknown and unknowable.

Why not? If someone is interested, what harm is that to you? ^^"

The ending cutscene is Rabiane defeating the Observerand leaving the Cathedral but it's not confirmed if she actually punched it out or it just left after Rabiane beats the abortion mini-game since she still got slugged. It's like the instant kill from the Father without the powder except she got instant mindcontrolled.

That interpretation lines up pretty well with how the demihumans also draw an eye on the belly in their scene

One of the basis of my theory that the minions of the Father can kinda feel his presence and have some sort of idea of what he wants, probably shown them in visions and dreams, however, they are, especially the demihumans, too dumb to understand the exact want of the father. Hence why everyone fails to produce any better than abomination broodlings, except when Lilia and Haanja themselves unwittingly provide the perfect conditions without the interference of all the dumb cultists messing the plan up. XD
(but those same conditions are the ones they need to kamehameha him back to kingdom come :p)

Btw a little thing I've been thinking about before regarding this screenshot / the layout of the 1st game map. Rabiane approaches this cathedral from the back door and never goes to the bridge seen on the right of the picture, while Lilia goes to her cathedral from the bridge side. It could be a theory that the 1st and 2nd games are neighboring areas with the cathedral being in the center of the 2 areas and Rabiane / Lilia simply approached the place from 2 opposed sides.
(ofcourse this is all if we reeeeeeally wanna connect the games...in the end these two areas might be just 2 far off lands and the 2 games might simply have nothing to do with each other.)

Hell, here is another interesting idea. The observer is seen in the 2nd game as an easter egg in a dark corner. What if it's not an easter egg. Lilia does pray to the observer to get a special item...here is a new interpretation...
What if the slug you find after kicking the ass of the father is not the father...but the observer. Making the 2nd game a prequel....
Lilia and Haanja does win, and they do beat the Father...but Lilia is actually a devotee of the Observer and gives birth to it after the Father is gone, leading into the 1st game. The observer might be weaker than the father hence why it was in a dark corner hiding while the father was dominant.
Lilia is also not controlled at the end unlike Rabiane. She can freely choose to kill it, or lovingly embrace it and copulate with it.
But of course this is just another plausible theory based on a hidden Easter egg. *shrugs*

Observer's not fully manifest like the Seere's on the way to him. Otherwise, he'll just grab Rabiane like what Father tried to do as a desperation attack.
