Does anyone know how to unlock the last scene in the Highway Gallery? It just says Use an item in a particular location

You get scenes with his pigs banging Leila, breast growth due to their parasites, Pig Man milking Leila between those scenes, and then Leila milking herself to reduce super growth. If you lose your infertility blessing (losing the dream fight to save the pregnant woman or getting the womb tattoo from the cultist in the Cathedral Underground), you unlock the pregnancy debuff status and the pregnancy content for demihumans, the cultist, the priest in Northeast Village, the dream fight to undo Leila's pregnancy, AND the Pig Man fight.

Check the map, on the road to Demihuman cave have the boss area , use the Scivias which use on Final Boss
Go right, but instead of the door to the Library, take the stairs to the lower floors. You get to fight a creepy portrait boss with tentacles that wants to turn Leila into +18 fanart and force people to pay Patreon so they can see it (all of that treasure is because the portrait only accepts cash and ingots)

We absolutely need a talented artist with way too much time on their hands to make a mod that turns Lelia into Rabiane from the first game, man. That would be so goated.

What you say is kind of a variation on what I say. It's still a last ditch effort by a weakened enemy. Weather it happens in a dream world or reality or perhaps in the otherworld is something that might be answered in a dlc. *shrug*
Similar to Tina the Swordsman, I agree with your sentiments. I would imagine that only applies when Leila has the infertility blessing, because otherwise she gives birth to an abomination broodling... then again it can be a scene with the drunkard banging a pregnant Leila and the scene staying there with the implications known to the player.
TBF neither of the games has that kind of content. Your nun either becomes a sex slave, experiences a horrible guro death, or becomes a mother of broodlings/pigs/otherworlders.
Sequel so good that the 6-year old prequel shot up to 1st placethis couple of days.

Em, no? Just spam your buttons while removing session, just like you would escape a grab and you good
The parasite not cause any trouble though. 
Except it required to open the village door (in case you didn't want try hard to defeat him there) so you must keep it until open the door.

Not sure this is possible but opening save menu and save whenever we want is what I'd want to do. Repeating parasite loss just to view scene in full sequence is nuisance. Maybe we can re-watch the final ending scene too if this option is possible.

The first game takes place in a land called Kessaria (apparently a city in Turkey IRL) and the second game in Arcezon, plus the fact that both final bosses seek to be born. Now, accepting the premise of this game being the prequel and ignoring the different names for the time being, take the following scenario:
1. Abomination Broodlings are killing the population in the countryside either by eating, straight up murder and torture, or using them as breeders. The latter issue is bad enough that many, if not ANY, children conceived between humans end up in the conception and birth of an abomination broodling that repeats the process in its place of birth. Villages are either quickly wiped out or experience a slower death in a year or even months due to nonexistent human births.
2. Local armies are not available due to isolation or lack of human power; perhaps the big cities are keeping their armies away to protect themselves, and only an ignorant fool would go and try seizing the abandoned countryside. The poor villagers still get taxed because fuck them.
3. Hanya and Leila are sent to deal with an outbreak in this particular town and its countryside, and to find out what the hell is going on. They are not the first to arrive because there were clues about the local sisters trying to fight off the otherworlder, or because those sisters were the first response. The events of the game happen, and Leila and Hanya lose because of the last-ditch effort from the Otherworlder (the last slug).
4. Years pass, and the problem has spread further to the point that the Sisterhood is running low on resources, maybe they lost some of their best, like Leila and Hanya. The bunch of petrified nuns in Kessaria's Cathedral can be used as an example.
5. Rabiane would be a very competent warrior nun, but one with more limited resources at her disposal because some of the core magical teachings have been lost in the years since the ending of this game. She has to clean up both the mess created by her predecessors and the outbreak connected to a new Otherworlder.
