Where should I use the item? And even if she gets pregnant by a ruffian or a drunkard she will still get pregnant with an otherworldly being?
There is a location in the north on the map with two pillars. Go there, use the item between the two pillars.

But yeah, no matter who impregnates her, she will always gives birth to a otherworldly being... which is kinda sad, but it makes sense if you follow the story. One only wonders, if that is true with the curse, why is mankind still around and not died-out already?

I don't know why I was hoping that if a human got her pregnant it would add another conflict LoL

Because the problem is restricted to Arcezon, and it has not reached the scale of Kessaria. In the first game, the only living humans left in Kessaria are Rabiane and the cultists; the rest of the " people" are the pigmen and the villagers mutated by the parasites. Were it not for the different locations, this game could be considered a prequel to the first one, since they share a lot of monsters and the Observer/Incubus even appears briefly in the Cathedral.

You simply can't get pregnant until the cathedral priest catch leila (Let them beat you up), where they replace leila's blessing with a womb crest. making sure you have a chance to get pregnant.
actually, you can get it sooner.
On the west side of town there is a pregnant woman who needs said abomination removed. You have to go and fetch the ingredients to do a dreambattle with a weird fetus like boss covered in... Slugs? I guess?

If you lose that fight, the blessing falls off.

You're warned before the fight that while you can't take physical damage from the dream battle, it can still affect your mind- and the blessing falls off if the holder desire pregnancy "even for a moment".

I kinda suspect parasite infection can have a similar effect, but I have yet to test it.

This one is incredible. Especially demihuman/pig scenes, though i'd love to see much more content with them, more captivity loop interactions... So sad, that there is very few games like that.

Update on my English retranslation patch (07/26)

View attachment 5080166

I managed to translate everything before the Northeast Village dungeon, the preview patch (see attachment) now includes all dungeons and events up until the attack by demihumans event + the Stomach Cave dungeon. Gallery entries are still not translated, as my priority is on translating all the dungeons and events first.

The patch won't prevent you from going into the Northeast Village dungeon and finishing the game, but you'll begin to see strings starting with 3-digit numbers, which are the original English translations.

I've tried my best to debug by playing the game over and over. However, if you find any lines (prior to the Northeast Village) starting with 3-digit numbers, having weird line breaks, and/or being cut off too early, do let me know.

A really well polished game! It was fun, I'm not really into goro but I viewed it as an echi dark souls game and had a lot of fun. There are many scenes I still enjoyed as well, the animations/models/environments/music/story its all very high quality. I wish other games of this genre would implement gallery of defeated enemies; like they did in this game, it is a large quality of life feature. FYI omg and the final boss is actually really challenging it took me like 10 tries and i had maxed out everything and tons of potions xD

It doesnt. Leila simply mutates due to the parasite after accepting the "Holy Body". Once the Blessing of Infertility is gone, Leila can be impregnated by other creatures, including the mutated villagers, which catches the attention of the priest. He uses the pregnancy to summon the Otherworlder, but ends up turning the fetus in Leila's body into a lesser replica similar to the one she births when she is mind controled in the Cathedral Underground.

I'm working on a dark skin Lelia mod. Using the brunette skin as a base, it's literally just a layer of brown color over all exposed bits of skin. I am about halfway through all the portraits but haven't touched the sprite yet.

It's a little rough, particularly around the 'seams'.

I experimented with white haired Lelia too and that it looks great too but gosh it'd be a lot of work...

You can also lose it if you allow a certain amount of events occurs while held captive by the demihumans. Their shamans cast a spell in the event that breaks the seal.

so there’s three confirmed ways you can lose it

I have been repeating the scenes with obeying the goblins, but no shaman appeared. How did you do it?

Can you share a program for unpacking and packing files?
I want to translate into another language...
