Hell, majority of professionally translated(like JAST USA) or dedicated fan translators(like Majide Koi series) games and novels are it. And competition among the genre is high, you don't even see most of the vanilla shovelware(or just plain and short) stuff translated because translators only have so much time, and spend it on top shelf.
So yes, not only english, but japanese top is mostly vanilla
As for the why it seems like it's only NTR games here... Well, they are easy to translate with MTL, easy to make - same tropes, same engine, different art. And ntr lovers are more active in request forums, I guess? Maybe because the demand isn't satisfied elsewhere, so they get their fix here.

Did you put Game.exe or Game_En.exe? If you want to play English, usually you put game_en.exe as the executable file in Joiplay. Or just grab the AI TL patch in DLsite as it is usually free to download

I did the Game_En.exe file first, when I found out there was no translation to English, I tried the other file.

I then went to download the patch on the dlsite, but nothing happened. The game remains in Japanese. Maybe this is just some weird mobile/joyplay problem.
