NTR as japanese and shorten word take various form but all have in common the betrayal of the partner (the partner neither need to be engaged, just close is enough).

The reality is, whether correct or not, many people will loudly yell about a game needing the NTR tag if any female character exists that isn't 100% loyal to their chosen male self-insert, even for female protag games. So it's better to be on the safe side

Difficult is a big challenge but still achievable, normal is also good, nightmare is something I personally wouldn't touch unless you're a god.
