Load up the previous version and save outside the hotel during the day, then try the save on the new version. If it's like older versions with this same issue, you should be able to continue from there.The old saves don't work for 0.46.

My 0.45 saves worked for 0.46, but I still feel your pain because I've had the same thing happen with other Renpy games where my saves didn't work but other players said their saves work fine.The old saves don't work for 0.46.

One suggestion that has worked for me in the past is to try all of the old autosaves first.
Can someone post a save of this new version, I find it very difficult to navigate if you start from the beginning
How do you trigger this one? Have done all the other therapy sessions, but Lil never comes up
On Tuesdays and Thursdays where Lily isn't working, (starting from an old save), there's a new option to have Lily join in paid therapy at 17:00. If you request it on a Friday, it'll be scheduled for Saturday.

One of the criticisms of the game that I agree with is that there's a bit of randomness at the end game which will sometimes lock up some scenarios that are added.
While you may have maxed the Lit Teacher's scenes (with MC, with Mom and MC, with Principal, with PE Teacher in Library), do you have enough points with Lil who is a bit more complicated (with Mom and Jill, with Xshop owner, MC including the newish Anal option, at the Strip club, with step sister)
I think sometimes the (you're too depraved) flag locks some scenes. For instance, in one of my saves, I'm locked out of threesome with Jill and friend at the motel room for some reason, while another save doesn't have that issue. Also, for this current version, I think my saves are locked out of the new scenes with the PE teacher in the gym.
On your question about the HJ, it's just a new angle on the same HJ scene for incest confession.

On Monday try to go to the afternoon class and get a bj from the lit teacher, it will trigger the scene where Lilly catches you and it will have a dialogue about her therapy session, and you will have the third option when requesting to have a therapy session with Lilly.
