As for the pregnancy question - only the developer can answer it. I personally do not care about this issue.

It's up to you, but a number of scenes are actually changed at the beginning of the story. They don't seem to break the game if you use the old saves, but I would personally start the game from scratch.

No, you invite aunt to stay with your step family.

That took quite a while for me too, but in essence, you need to have:
- creampied both (preferrably analled Jill as well from memory...)
- had group sex with both (with other fam members etc.)
- stay with Jill in the same room
- invite aunt to stay in alternative room
- wait for Jill to come back from work IN THE CARPARK*, any night that she's working will do. Just make sure you wait from 2AM onwards.

Hmmm it's been a little while but... do you remember if you joined your step fam for dinner enough times? I believe the conversation needs to come up first.
THEN you talk to the aunt about the same... it should then open the option up for you.
At least that worked for me!
Edit: pls ignore this as CbunnySwtor has the best guide - I totally forgot about the Lily bit. But if you said that you've triggered *all* other scenes, which should also include a Mary/Lily scene, then this must've already happened for you, no?

Invite LilSister to dinner at stephouse when it's dinner time at the diner and aunt is staying in your motel room and she'll catch you going. that worked for me.
Start by telling Lily (red hair sister) about the step family. Progress with the aunt till you are able to sleep with her (banging isnt required, just be allowed to sleep together).
Eventually, the step sisters will ask you to invite Lily to dinner at the step family's place. Once you've done it once, do it while the Aunt is visiting. She'll stop you and go with you.
At some point in all that, you'll be given a room in the step family house. Once you've done all that you can offer your room to the Aunt when she visits. At that point, screw with her during meditation and, assuming you've tapped the her, the step mom will interrupt and fun ensues.
Progress far enough with the cop at the party and/or motel (giving her an orgasm anywhere else is like a free pass). Then you'll be able to make lewd suggestions to her in the Detectives Room. She'll drag you off to the storage room then.
