I just finished Season 1 and wanted to know who was that at the end with Bella? Are we supposed to know?

Let's not be so hasty. We don't know yet if she's Bella's sister.
She could very well end up being an adopted childhood friend former step-roommate.
I've played this game like 5 times and I still can't figure out what happened to Summer other than she disappeared 5 years ago and I don't know if I've missed something in the threads or in the game, maybe they just haven't said what happened to her yet. If someone knows anything please tell me thanks. If they didn't maybe it will come out in Season 2

I am not sure if even the Ocean has already decided what happened to her. She died / was kidnapped / was punished for her relationship with a commoner (Nika)

If there is, I either didn´t see it or don´t remember it. I had the thought that it could be her sister, but even that was based on something I vaguely remember her saying. The picture of the two of them is whta makes Nika ask Bella about her sister the first time he's over her house to study. Because of her reaction, and the way Ayua reacts about Nika asking about Bella's sister, it's safe to assume something bad happened.
Because she appears to be in home care, and there's all those missed presents, it's be really weird if it was a stranger ...or former roommate.
And there's enough life support gear there, to consider that if she's in home care, they've set up for the long term. Bella talking to her knowing show won't get a response, the unopened presents etc, wishing she could see Nika's eye's someday, all suggest she's not waking up.

She was sent to that briefly mentioned christian summer camp to get her away from Nika and was diddled. After Nika finds out he enters a fit of rage and his own accident happens.(this is my own speculation)
I can't remember if the camp was mentioned in Summers gone but I think it was mentioned in Where It All Began during the breakfast scene.
